Research gaps regarding Cambodia's mental health systems and services represent a major hurdle in establishing evidence-based mental health policies and operational practices. Addressing this impediment in Cambodia necessitates the implementation of effective research and development strategies, grounded in locally-prioritized research. The potential for mental health research in low- and middle-income countries, like Cambodia, demands a deliberate framework of research priorities to optimally guide future research investments. International collaborative workshops in Cambodia, on mental health service mapping and research priority setting, contributed to the development of this paper.

Cancer immunotherapy, particularly immune checkpoint blockade, has sparked a revolution in therapeutic strategies by reinvigorating the host's immune response. In contrast, the effectiveness is not consistent, and only a small amount of patients achieve lasting anti-tumor responses. Consequently, novel strategies aimed at enhancing the clinical efficacy of immune checkpoint therapy are urgently required. The dynamic and efficient nature of the post-transcriptional modification process N6-methyladenosine (m6A) has been empirically verified. This entity is instrumental in a wide array of RNA procedures, from splicing and transport to translation and the degradation of RNA. By demonstrating the substantial role of m6A modification, compelling evidence underscores its importance in immune response regulation. The observed results could serve as a springboard for strategically integrating m6A modification targeting and immune checkpoint blockade in cancer therapies. This review compiles the current body of knowledge on m6A modification in RNA biology, focusing on the latest findings about the complex mechanisms through which m6A modification affects immune checkpoint molecules. In addition, acknowledging the essential part of m6A modification within the context of anti-tumor immunity, we analyze the clinical significance of targeting m6A modification to improve the efficacy of immune checkpoint inhibitors in cancer control.
Various types of ailments have found widespread use for N-acetylcysteine (NAC) as an antioxidant. This research evaluated whether NAC treatment could affect the course and prognosis of systemic lupus erythematosus (SLE).
Eighty patients with systemic lupus erythematosus (SLE) were randomly assigned to one of two groups in a double-blind, controlled clinical trial. Forty patients received N-acetylcysteine (NAC) at 1800 milligrams per day, divided into three doses spaced eight hours apart, for three months. Forty control patients received standard treatments. Prior to treatment commencement and following the conclusion of the study period, laboratory assessments and disease activity, as evaluated by the British Isles Lupus Assessment Group (BILAG) and the Systemic Lupus Erythematosus Disease Activity Index (SLEDAI), were established.
A statistically significant improvement in BILAG and SLEDAI scores (P=0.0023 and P=0.0034, respectively) was observed in patients treated with NAC over a three-month period. Three months post-treatment, NAC-treated patients had significantly lower BILAG (P=0.0021) and SLEDAI (P=0.0030) scores than the control group. The NAC group, after treatment, demonstrated a statistically significant decrease in disease activity throughout various organs, as determined by the BILAG score (P=0.0018) compared to the baseline. This decrease was significant in mucocutaneous (P=0.0003), neurological (P=0.0015), musculoskeletal (P=0.0048), cardiorespiratory (P=0.0047), renal (P=0.0025), and vascular (P=0.0048) complications. Post-treatment analysis demonstrated a considerable increase in CH50 levels within the NAC group when compared to their baseline levels, a difference that was statistically significant (P=0.049). Dissemination and Implementation Science (DIS) unique methods and priorities are not factored into the existing grant review standards. To assess DIS research proposals, the INSPECT scoring system, drawing on Proctor et al.'s ten key elements, employs ten criteria. Our DIS Center's approach for evaluating pilot DIS study proposals involved a customized INSPECT adaptation, coupled with the NIH scoring system.
With the aim of incorporating diverse DIS settings and concepts, we adjusted INSPECT's parameters, specifically by including the detailed procedures of dissemination and implementation. For the evaluation of seven grant proposals, five PhD-level researchers proficient in DIS, at an intermediate to advanced level, were trained to employ INSPECT and NIH criteria. INSPECT overall scores are evaluated within the range of 0 to 30, higher scores denoting better performance; in contrast, the NIH overall scores are rated on a 1 to 9 scale, with lower scores reflecting greater quality. Each grant underwent independent assessment by two reviewers, followed by a collective discussion involving their individual experiences, applying both criteria for a comprehensive proposal evaluation and conclusive scoring. In order to gather additional perspectives on each scoring criterion, a follow-up survey was sent to grant reviewers.
Reviewers' evaluations demonstrated a substantial variation for both INSPECT and NIH scores. INSPECT scores averaged between 13 and 24, while NIH scores were between 2 and 5. Effectiveness and pre-implementation strategies were better evaluated by the NIH criteria, owing to their broad scientific scope, as compared to proposals that tested implementation methods.
